rfc: simplify punctuation settings

All,

currently speakup has 3 different groups of punctuation levels:

- speakup punctuation level
- reading punctuation level
- synthesizer punctuation level

The speakup and reading punctuation levels are able to be set by the
user from the keyboard, but the synthesizer punctuation has to be set
from a sys file.

I have no idea what the reading punctuation level is or why it is
separate from the speakup punctuation level.  What does everyone think
about combining the reading punctuation level into the speakup
punctuation level and using the keys that adjust the reading punctuation
level to control the synth punctuation level?
