A Sumary for Speakup guide

Hello:

I missed a sumary/table of contents when consulting the SPKGuide, so I 
decided to make one. I'm attaching my modified SPKGuide in case you want 
to include it in the main Speakup distribution. For this message not to 
exceed the list size limit, I'm stripping out all the contents of the 
guide except for the heading where I made my changes.
