I just keep fetchmail running in the background by putting a "set daemon
300" line in my .fetchmailrc and then running fetchmail from
.bash_profile. This fetches mail from all the servers defined in my
.fetchmailrc every 5 minutes.

To get exim working with your smarthost make a smarthost router with the
name of your smart host in the config file. I believe Debian and maybe
others ask you how you want your mail server to run and you can choose
to use a smart host. Doing it this way makes all the necessary changes
to the exim configuration.  On Debian you can do "dpkg-reconfigure
exim4" and it will ask you the questions again.
