I can only think of two possibilities: 1.) Formatting did not get accomplished correctly and you're running out of space on /usr or 2.) The CD ROM is bad. If it's the latter I will happily send another with apologies. If it's the former, we'd have to go back with another round. But, that would give us the opportunity to get it right.
